Nurse Practitioner

Medical Nurse Practitioner – Skilled Nursing & Assisted Living

Bluffton, SC | Full-Time
Salary: ~$110,000–$130,000 annually (based on experience)

About the Opportunity

A well-established healthcare organization is seeking a Medical Nurse Practitioner to provide comprehensive care to residents across skilled nursing and assisted living communities in Bluffton, SC.

This is an excellent opportunity for an experienced NP who enjoys caring for the senior population and values building meaningful relationships with patients, families, and care teams. The position offers a collaborative clinical environment, a high level of autonomy, and the opportunity to work closely with organizational leadership.

What You'll Do
Round on residents and provide ongoing medical evaluation and care.
Evaluate changes in condition and develop appropriate treatment plans.
Assess new admissions and establish individualized plans of care.
Diagnose and manage acute and chronic medical conditions.
Order and interpret appropriate labs, diagnostic testing, and imaging.
Prescribe and manage medications as clinically appropriate.
Monitor patient progress and adjust treatment plans as needed.
Collaborate with physicians, nursing staff, therapists, specialists, and other members of the care team.
Participate in interdisciplinary care planning and case discussions.
Communicate with residents and families regarding diagnoses, treatment plans, and ongoing care.
Maintain accurate and timely clinical documentation.
Help promote quality outcomes, patient safety, and continuity of care.


What You'll Need
Master's or Doctoral degree from an accredited Nurse Practitioner program.
Active, unrestricted Nurse Practitioner/APRN license in South Carolina or ability to obtain one.
Current national board certification.
Current BLS certification.
Clinical experience managing acute and chronic conditions.
Strong assessment, clinical judgment, and decision-making skills.
Excellent communication and interpersonal skills.
Preferred Experience
Experience in skilled nursing, assisted living, long-term care, geriatrics, primary care, or a similar setting.
Experience caring for medically complex or senior patients.
Familiarity with common geriatric conditions and chronic disease management.
Experience with electronic health record systems.
