Position Overview
The Resident Intelligence RN is an on-site Vitability nurse responsible for turning continuous monitoring data into clinical action and ensuring concerns are followed through to resolution. The RN works inside a partner facility but is employed by Vitability and supports Vitability-enrolled residents. This is not a facility staffing position and does not include taking assignments or passing medications.

Key Responsibilities

• Review daily monitoring data for changes in heart rate, respiratory rate, bed exits, and repositioning patterns.
• Assess residents at the bedside and repeat vital signs when clinically appropriate.
• Conduct and facilitate daily telehealth visits with Vitability providers, presenting clinical findings and assisting with resident assessment.
• Prepare SBARs and communicate meaningful changes in condition to Vitability providers and the facility clinical team.
• Follow provider recommendations through completion and confirm resident outcomes, closing the loop on identified concerns.
• Coordinate with facility nursing, practitioners, therapy, wound care, dietary, and other disciplines as appropriate.
• Complete CCM activities including care-plan review, medication reconciliation, laboratory review, and identification of care gaps.
• Support CoCM services, including behavioral health assessments, registry management, psychiatric case review, and follow-up on recommendations.
• Identify medication-related risks and support medication optimization and gradual dose-reduction reviews.
• Accurately document clinical activities and time associated with RPM, CCM, and CoCM services.

Qualifications

• Current unrestricted RN license (required)
• Strong clinical assessment and communication skills (required)
• 2 years Nursing experience (preferred)
• SNF, long-term care, geriatrics, care management, or acute-care experience (strongly preferred)

The goal of this position is simple: identify meaningful changes early, get the right people involved, and stay with the resident until the loop is closed.
