Deer Meadows Home Health is seeking a skilled and compassionate Registered Nurse (RN) who’s ready to provide high-quality care right where it’s needed most—in the patient’s home. Responsibilities
- Conduct initial and ongoing comprehensive assessments to identify patients’ clinical needs
- Develop individualized care plans and revise them as conditions change
- Deliver skilled nursing care, including medication administration, wound care, and disease management
- Provide patient and family education to support health goals and self-management
- Collaborate closely with physicians, caregivers, and interdisciplinary team members to ensure coordinated care
- Supervise and evaluate home health aides every two weeks
- Participate in the rotating on-call schedule as needed
Qualifications
- Graduate of an accredited nursing program
- Current Registered Nurse (RN) license in Pennsylvania
- Valid CPR certification
- 1–2 years of acute care experience in a healthcare setting
- Reliable transportation and a valid driver’s license
- Preferred: Bachelor’s degree in Nursing and home health care experience
