Responsibilities

  • Provide personalized, one-on-one therapy sessions in the patient’s home
  • Develop and implement customized care plans that support mobility and recovery goals
  • Assess home environments and recommend safety modifications
  • Track and report patient progress, communicating with physicians and the care team
  • Collaborate with nurses, caregivers, and other clinicians for holistic care delivery
  • Travel to patient homes throughout Philadelphia County
  • Supervise PTAs in compliance with state regulations and agency policies

Qualifications

  • Bachelor’s or Master’s degree in Physical Therapy from an accredited program
  • Active Pennsylvania Physical Therapy license
  • At least 2 years of experience as a Physical Therapist (home health preferred)
  • Current CPR certification
  • Reliable transportation and a valid driver’s license
  • A passion for empowering patients through home-based recovery
