Job Description

Division/Department:  Human Services – Medical Services

Location: Min No Aya Win Human Services Center & Center for American Indian Resources

Reports To: Medical Clinic Coordinator 

Probationary Period: 90 days – Full Time; 120 days – Part Time

FLSA Status: Exempt (Salaried)

Classification: Child-Related CA

Driver’s License: No Driver’s License is required.

Rate of Pay: $77,589.34 to $87,288.01

 

Responsibilities

  • Conduct comprehensive nutrition assessments for patients, including reviewing medical history, current health conditions, dietary habits, and lifestyle factors.
  • Develop personalized nutrition care plans based on the patient’s specific needs and goals. Provide one-on-one counseling on topics such as weight management, diabetes, cardiovascular health, gastrointestinal issues, food allergies, and other chronic conditions.
  • Educate patients on the relationship between nutrition and health, and provide practical advice on meal planning, food choices, portion control, label reading, and cooking techniques.
  • Support patients in adopting and maintaining positive dietary behaviors by using motivational interviewing techniques, goal setting, and self-monitoring strategies.
  • Schedule follow-up appointments to monitor patient progress, reassess their needs, and adjust nutrition plans accordingly to ensure ongoing success.
  • Work collaboratively with diabetes team, providers, nurses, and other healthcare professionals to coordinate patient care and provide a holistic approach to managing patients’ conditions.
  • Maintain accurate and up-to-date patient records in accordance with organizational policies and healthcare regulations. Document patient progress, interventions, and outcomes in the electronic health record (EHR) system.
  • Facilitate weight management programs for youth and adults.
  • Participate in health fairs, blood sugar screenings, offer grocery store tours, cooking demonstrations, and other community outreach activities to promote nutrition education and raise awareness about the importance of healthy eating.
  • Participates in the planning, development, marketing and implementation of nutrition, diabetes prevention, and diabetes-related programs for clinic clients or groups, staff, and for community outreach.
  • Assist with maintenance of the American Diabetes Association Recognition Program and the Indian Health Service Special Diabetes Program for Indians grant applications and Best Practices as directed.
  • Participate in Continuous Quality Improvement activities as directed.
  • Manage or assist with the Diabetes Support Group and Diabetes Newsletter.
  • Works in collaboration with the Medical Clinic Coordinator, Medical Director, and Diabetes Program Coordinator on tasks related to the program.
  • Assumes responsibilities for personal, professional development and maintains clinical and educational competency.
  • Participates in diabetes team and clinic team meetings.
  • Appropriate/Professional work attire is required.
  • Must attend all mandatory department trainings.
  • Ensures confidentiality of financial records and all records for employees and/or clients.
  • Displays a responsive and professional manner in promptly responding to all requests, complaints, and problems.
  • Recognizes that each employee is a representative of the Fond du Lac Band of Lake Superior Chippewa and is responsible for demonstrating courtesy, respect, and sensitivity to the needs of all others, including visitors and co-workers.
  • Represents the Fond du Lac Band of Lake Superior Chippewa and the department in a positive and professional manner in the community.
  • Required to maintain proper attendance including reporting to work on time in accordance with applicable policies.
  • Maintains a clean and organized work area.
  • Due to changes and modifications in the job from time to time, employees are required to be flexible and assume other responsibilities assigned by management as deemed necessary.

Qualifications

  • Registered Dietitian, Licensed Dietitian in the state of Minnesota is required.
  • Certified Diabetes Educator (CDE) is required; if not certified at the time of hire employee must obtain certification within 24 months of hire, and must maintain certification for the duration of employment.
  • One year of experience in clinical nutrition counseling is preferred.
  • Knowledge of computer applications to include word processing, data bases and/or data entry, Spreadsheets, and Microsoft Outlook is required.
  • Knowledge of Native American communities, culture, customs, and beliefs is preferred.
  • Experience working with Native Americans clients, communities and is able to demonstrate cultural sensitivity is preferred.
  • Ability to communicate effectively orally and in writing is required.
  • Ability to work independently and establish priorities is required.
  • Attention to detail and accuracy is required.
  • Ability to establish professional and harmonious working relationships on all projects and with all parties involved.
  • Subject to drug and alcohol testing in accordance with the Fond du Lac Reservation’s Personnel Policies.
  • Subject to pre-employment and annual background checks.
  • Some travel may be required.
